New Patient Registration

If you would like to register with the practice please either download and complete the forms below.

You must complete both the GMS1 form and the New Patient Registration form to be able to register with the practice.

If you are taking any medication currently it is very helpful if we have a copy of a prescription when you register.

You will need to provide two forms of ID, one photographic (i.e. passport or driving license) AND either a utility bill or bank statement as proof of your home address. We are unable to process your registration without this.

Please come to the surgery with your completed forms and your ID OR please scan your completed forms and copies of your ID (passport, drivers licence and utility bill dated within the past three months for proof of address) to file and send them in via email to moreton.doctors@nhs.net.
