New Patient Registration

If you would like to register with the practice please download and complete the forms below.

You must complete both the GMS1 form and the New Patient Registration form to be able to register with the practice.

• GMS1
• New Patient Registration – adults (age 18 and over)
• Child and Young Persons New Patient Registration Form (under 18)

You will need to provide two forms of ID, one photographic (i.e. passport or driving license) AND either a utility bill or bank statement as proof of your home address. We are unable to process your registration without this. Once you have completed the Registration Forms, we can make an appointment for you to see one of our practice nurses for a new patient check.

Prescriptions

It is practice policy following CCG guidance that repeat prescriptions are issued for a 28 day/one month period only except for a minority of medications which are prescribed 3 monthly. Please check with dispensary if in doubt.

If you have a medication related query please try to speak to the dispensary team in the first instance who may be able to help.

Allow 3 working days between ordering and collection.
