New Patient Registration

If you would like to register with the practice please download and complete the forms below.

You must complete both the GMS1 form and the New Patient Registration form to be able to register with the practice.

• GMS1
• New Patient Registration – adults (age 18 and over)
• Child and Young Persons New Patient Registration Form (under 18)

You will need to provide two forms of ID, one photographic (i.e. passport or driving license) AND either a utility bill or bank statement as proof of your home address. We are unable to process your registration without this.

Please come to the surgery with your completed forms and your ID OR please scan you completed forms and copies of your ID (passport, drivers licence and utility bill dated within the past three months for proof of address) to file and send them in via email to moreton.doctors@nhs.net.

Prescriptions

It is practice policy following CCG guidance that repeat prescriptions are issued for a 28 day/one month period only.

If you have a medication related query please try to speak to the dispensary team in the first instance who may be able to help.

Allow 4 working days between ordering and collection.
