Registering with the surgery

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

If you would like to register with the practice please either download and complete the forms below or use the online ‘register with your GP‘ service.

You must complete both the PRF1 form and the new patient registration form to be able to register with the practice.

If you are taking any medication currently it is very helpful if we have a copy of a prescription when you register.

We would if you could provide two forms of ID, one photographic (i.e. passport or driving license) and either a utility bill or bank statement as proof of your home address. If you do not have any valid ID or proof of address please speak with us.

Please come to the surgery with your ID or send in scanned copies to moreton.doctors@nhs.net.

Please read the following documents carefully as part of the registration process:

Prescriptions

It is practice policy following CCG guidance that repeat prescriptions are issued for a 28 day/one month period only.

If you have a medication related query please try to speak to the dispensary team in the first instance.

Please allow 4 days for repeat prescription requests to be fulfilled.

If you are running short of medication please gain a months’ worth from your current practice, before applying for registration at Mann Cottage Surgery.
