Long Term Condition reviews

  • You have been directed to this page as you have been sent an invite for your long term condition review.
  • A long term condition refers to any health problem that requires monitoring and we aim to align our patients annual reviews to their month of birth.
  • Each patient who requires blood test monitoring will require an appointment with one of our health care assistants and will have a minimum of blood tests and a blood pressure check, but you may be asked some other health screening questions e.g. smoking status at the same time. If you then require a review with a clinical pharmacist, specialist nurse or a GP, our health care assistant will give you a slip of paper to take to reception to book the necessary follow up.
  • Patients who have a diagnosis of dementia, learning difficulties or who are housebound, will be contacted directly to arrange their annual reviews, rather than being sent a text, email or letter to invite them
  • Please contact our reception team to book your Long Term Condition (LTC) appointment(s).
  • Below is a list of conditions which qualify as LTCs as an approximate guide as to what you can expect from your annual review :-

COPD (chronic lung disease, which is not asthma)Annual face to face review with nurse specialist
AsthmaComplete annual Asthma Control Test questionnaire sent out via text or annual review with nurse specialist. All children to have face to face review.
Inherited high cholesterol (familial hypercholesterolaemia)Annual bloods and BP
